CITY RESOLUTION NO. 126

WHEREAS, ALUFORM, cognizant of the paramount need of its employees to prompt medical attention and expertise, hereby seeks to establish an affiliation with SJMC in the admission and provision of emergency-related services to its employees in accordance with the specific payment scheme;
WHEREAS, SJMC is a corporation duly engaged in the business of providing medical and hospital services to the public;
WHEREAS, SJMC has and maintains substantial capital, equipment and materials, as well as the manpower with the skills and expertise, necessary in the conduct of its business;
WHEREAS, ALUFORM and SJMC desire to affiliate for the purpose of carrying out the said emergency needs of the employees SJMC’s facilities following a special payment scheme.
WHEREAS, the City Legal Officer has certified that the Memorandum of Agreement is in order and not contrary to existing laws, rules and regulations.
NOW THEREFORE, BE IT RESOLVED AS IT IS HEREBY RESOLVED BY THE SANGGUNIANG PANLUNGSOD OF THE CITY OF SAN JUAN, AND BY VIRTUE OF THE POWERS VESTED IN IT BY LAW, in a session duly assembled, to authorize the City Mayor Francisco Javier M. Zamora, to enter into and sign the MOA with Aluform Pre-Fab Construction and Consultancy for purposes of affiliation with SJMC in the admission and provision of emergency-related services to its employees in accordance with the specific payment scheme.
RESOLVED FURTHER, that the authority, issuance and approval hereof is subject to certain restrictions and to all existing laws, circulars, rules and regulations.
